1. We complete a brief questionnaire about the person who needs the placement.
2. We provide a form for the client's physician to complete, which includes TB test information. This is the 'Physician's Report', which is required for all Placements into any RCFE: Assisted Living Facilities, Board & Care homes.
3. We consider all service requirements, area requests are budget parameters.
4. We will contact Facilities to provide information about the client, to get approval of conditions, and to check availability of accommodations. We will let them know You will be calling to set up an appointment for a visit. (when appropriate, we will set The appointments for you.)
5. We provide you with a list of 3-5 possible Facilities which meet the needs of the Client as closely as possible and have space available. You will set the appointments to visit the Facilities at your convenience, but we suggest as soon as possible as changes in availability occur daily.
6. If you need to see additional Facilities, we will search for others which also meet your criteria.
7. When you do like a particular Facility, call us so we can set up an appointment with the Facility Administrator to begin the process of Placement.
This includes:
a. Assessment of the Client by Facility personnel to determine
the level of care necessary at the Facility to meet the needs of the
Client.
b. At this time, cost is discussed which is based on any additional
services which may be needed.
c. The Physicians Report is given to the Facility, an admissions
agreement is signed and 'Move-in Day' is arranged.
8. Facilities require a 30 day notice if a Client decides to move. Moving can be very disruptive for the elderly and/or disabled and therefore it is best to find a 'good fit' initially and to try and resolve any problems before making another change. If a change of placement is necessary, Moonlite Transitions LLC will continue to assist.
